E-Form: MVS All Academic Team

Instructions

To be eligible for the MVS All Academic Team, you must meet the following criteria:

must have completed 9th Grade or higher;
must be a USA Swimming athlete member;
must have participated in an individual event in one of the MVS Championship meets or higher, such as Region VIII, Central Zones, Junior Championships, Senior Nationals, US Open, Olympic Trials, or Olympics;
must have participated in 3 additional USA Swimming sanctioned meets;
must have a minimum 3.2 grade point average on a 4.0 point scale;
must submit an Official Transcript or Report Card for the entire academic year.

This application form and your transcripts (or report card) must be received by the MVS Records Chair no later than August 30th. Late applications will not be accepted.
